What's My Level? FREE Follow-Up

What's My Level? is our FREE follow-up hormone and rate of bone loss testing program for all patients who have hormone therapy prescriptions filled through Madison Pharmacy Associates.

Hormone measurement and urine NTx bone loss testing begin at three months. Free follow-up testing continues annually thereafter for as long as prescriptions are filled through Madison Pharmacy Associates.

These FREE tests will help you:

• Avoid the side effects and health risks of hormone overdosing or underdosing.
• Achieve maximum relief from symptoms of hormone imbalance.
• Prevent bone loss and reduce the risk of osteoporosis.

The Tests

We will conduct free saliva hormone testing and free urine NTx Bone Loss testing three months after Madison Pharmacy Associates begins to fill your hormone prescriptions. These free follow-up tests will be conducted annually for as long as your prescriptions are filled by Madison Pharmacy Associates.

Have your healthcare provider complete the form with your current prescription information and fax it to Madison Pharmacy Associates. Our Restore^® specialists and registered pharmacists will follow-up with you and your provider to initiate a new prescription or transfer an existing prescription, and answer any of your questions.
